Service: Transforming Care

Our Transforming Care service is dedicated to improving the health and care of individuals with learning disabilities and autism. Our mission is to ensure they can live fulfilling lives within the community with the right support, reducing the need for hospital admissions.

We provide specialist support that focuses on individual strengths and empowers people to live more independently.

• Improving the quality of care for individuals with learning disabilities and/or autism.
• Enhancing the quality of life by ensuring access to appropriate care and community inclusion.
• Reducing inappropriate hospital admissions and minimising hospital stays by building strong community support networks.
